Appointment No-Show, Late, & Cancellation Policy

Policy
It is the policy of the South Jersey Skin Care & Laser Center’s practice to monitor and manage appointment no-shows, late cancellations, and late arrivals. The goal of the practice is to provide each patient with excellent care in a timely manner.

Cosmetic or aesthetic appointments require a 48-hour cancellation notice and general dermatology and surgical appointments require a 24-hour cancellation notice. A phone call, leaving a message, or sending an email are appropriate ways to communicate cancellations.

Definitions
“No-show” shall mean any person who fails to show up for their scheduled appointment.

“Same day cancellation” or “same day cancels” shall mean any person who cancels an appointment less than the required cancellation timeframe before their scheduled appointment type.

“Late arrival” shall mean any patient who arrives after the start time of the scheduled appointment. If the practice is unable to accommodate the late arrival, the patient will be classified as a “no-show”.

“Expected arrival time” shall mean the patient is expected to arrive no less than 10 minutes prior to the scheduled appointment time.

Procedure

A patient is notified of the No-Show, Late, & Cancellation Policy at the time of scheduling. A disclosure will be provided to patients at their appointment.

Appointments must be canceled in the appropriate time frame based on your appointment type as follows: cosmetic and aesthetic appointments require 48-hour notice and general and surgical dermatology require 24-hour notice.

In the event a patient arrives late as defined by “late arrival” to their appointment and cannot be seen by the provider on the same day, they will be rescheduled for a future visit, if available.

In the event a patient has incurred multiple documented “no-shows” and/or “same-day cancellations,” the patient may be subject to dismissal from South Jersey Skin Care & Laser Center.

A patient scheduled for a general dermatology appointment who no-shows or same-day cancels will be subject to a $30 no-show fee.

A patient scheduled for a surgical procedure who no-shows or same-day cancels will be subject to a $90 no-show fee.

A patient scheduled for a cosmetic or aesthetic appointment who no-shows or same-day cancels will be subject to a no-show fee as outlined in the Payment Policy below, the consent for treatment, and or quote forms signed prior to treatment.

Payment Policy Disclosure

Fees are non-refundable and non-transferable.

Patient responsibility

As a recipient of our services, you are responsible for the charges associated with the services you receive. You remain legally and fully responsible for your entire bill. Cash, check, debit cards, and most credit cards are accepted forms of payment. CareCredit is a line of credit between you and your creditor. At your request, we can provide you with information to apply for CareCredit.

Claims Submission

Submission of claims is a courtesy extended to our patients. If your insurance company does not pay your claim in 30 days, the balance will automatically be billed to you. Your insurance coverage is a contract between you and your insurance carrier; we are not a party to that contract. Obtaining referrals is your responsibility. You may be required to re-schedule your appointment if any necessary referrals have not been received by our practice prior to your appointment.

Co-payments and Deductibles

In accordance with your insurance plan and services provided, you are responsible for any and all co-payments, deductibles, and co-insurances.

Non-covered Services

Certain office procedures or services may not be covered or maybe considered “not medically necessary”, “experimental”, “cosmetic” or simply not covered by your insurance carrier. You are responsible for the payment of these services.

Non-sufficient funds / Returned Checks / Chargebacks
A fee of $30 will be charged for all returned checks and chargebacks.

A $100 non-refundable deposit is required at the time of scheduling, for select cosmetic services and esthetic treatments including Botox, filler, vascular, dermaplaning, HydraFacial, microdermabrasion, and chemical peels. Failure to show up to the appointment, provide the 48-hour notice, or arriving late to the appointment will result in forfeiture of the $100 non-refundable deposit. The forfeited deposit will be applied toward the provider’s time, resources, and room reservation.

Payment in full is required at the time of scheduling appointments for select cosmetic services and esthetic treatments including, QWO, Pearl, Fractional Laser, laser hair removal, laser genesis, microneedling, PRP, Titan, ViPeel, IPL, miraDry, and Coolsculpting. Failure to show up to your appointment, provide a 48-hour notice of cancelation, or arriving late to your appointment will result in a $500 forfeiture, or the cost of the procedure, whichever is less. The forfeited costs will be applied toward the provider’s time, resources, and room reservation.

Cosmetic consultations

Initial cosmetic consultations with our Cosmetic Consultant are complimentary, however, there will be a $100 non-refundable fee for subsequent consultations. If you do not show up for your appointment or fail to provide 48-hour notice cancellation a $100 non-refundable fee will be required to reschedule and will be applied to the consultation. If you fail to show or provide a 48-hour notice to the second consultation the $100 fee will be forfeited. The forfeited costs will be applied toward the provider’s time, resources, and room reservation. Failure to pay may result in loss of booking privileges and/or refusal to provide future service.

Medical provider cosmetic consultations

Cosmetic consultations with a medical provider (physician or physician assistant) are $100. In order to reserve your consultation with a medical provider, a $100 non-refundable consultation fee is required at the time of scheduling. This fee is for a cosmetic consultation. If a patient proceeds with any (cosmetic) treatment recommended by the medical provider within the consultation, the consultation fee can be used as a credit toward that cosmetic treatment. Treatment must be completed within 30 days of the consultation. If a patient does not complete treatment within 30 days, declines treatment, and/or is not a candidate for treatment, the $100 non-refundable consultation fee will be directed to the cosmetic consultation appointment. If a patient does not show for scheduled consultation or fails to provide the 48-hour notice, the $100 non-refundable consultation fee will be forfeited. The forfeited costs will be applied toward the provider’s time, resources, and room reservation.

Product consultation

The fee for a Product Consultation is a non-refundable $100 fee and is required at the time of scheduling. This fee is allocated toward the consultation and may be used as a credit toward products purchased on the day of your consultation. If you fail to show for your scheduled appointment time and/or do not provide the 48 hours required notice, the $100 fee will be allocated toward the provider’s time, room reservation, and resources.

Products

Product purchases are final. All open products are non-refundable. Unopened products may be returned within 7 days of purchase for an account credit. If you require assistance in determining your selection of products, our qualified licensed medical estheticians offer complimentary skincare consultations. They will gladly assist you in determining the appropriate selection of products prior to final sale.
